    Ppl just need to remember they are on the internet and what that implies :p

    You replace your fuel filter yet? I remember reading someone did theirs at 150k miles and it was full of gunk (jelly like :confused:). Mine was pretty clean but still a sizable diff when I changed mine.

    I really like the earlier theory that you have low fuel pressure and the 1st cylinder is the only one affected due to being the furthest down the fuel rail.

    Can you re-cap on everything you've done since the original post?

    Before taking it into a shop (recomended at this point), I would suggest the following:

    -Did you replace your fuel filter? Do this if you havent.
    -Check the neg terminal ground for good contact (and at least a gauge 4 wire)
    -Check the vacuum fitting on your fuel pressure regulator. It would be a hose from the regulator to a metal pipe between the upper and lower intake manifold and back to a hose that runs to the intake. Its connected to the large resonator box on my truck.
    -Check your PVC valve. If its bad, it can cause a vacuum leak and rough idle.
    -Check the neg terminal ground for good contact (and at least a gauge 4 wire)

    3" lift EIBACH coils w/ 5100's front, OME/ Dakar leaf springs w/ toytec adjustable shackles at 1.5" and 5125's out back. 33x10.5x15 KM2's Relentless DIY steel bumper.
    What's the proper way to test a PCV valve? Shake it to feel the wobble inside? Doesn't a tiny bit of oil in the PCV valve mean anything?

    OP I'm interested to find out what is gonna fix this issue, please keep us updated as to what you try. I'm battling a misfire except no codes being thrown.
    This weekend I pulled wires and ignition coils as the engine was running to see if any one I particular made a difference in the way it idled. Well cylinder 1 had very little effect on how it idled when I pulled the coil. I have new wires and coils on the way and praying it fixes my issue. My main issue is slightly rough idle and has been making a "thwarting" sound that is more noticeable when sitting in the cab and seems to be on the driver side. At first I thought it was a exhaust leak, but after having it looked at by my brother who is very mechanically inclined he thinks it's a misfire. I'm gonna try the coils and wires/ plugs this weekend and if that doesn't solve the issue I'll take it to my shop and have one of the mechanics look it over. I've never thought of a fuel filter being the cause but I'm gonna look into that and the negative grounds.

    Hope you get this resolved. I too have had this going on for 3 months or so. Truck runs great and starts up fine, just the slightly rough idle and "th th th th th th th" sound that has me searching for answers.

    Could have a cracked head. Mine did this and wasn't loosing much coolant but just a little and my head was cracked.

    The best way to check if there is a vacuum leak is disconnect the PCV valve from the intake manifold (attached right after the TB on the front side) and cap the vacuum connection and see if that changes the idle. My truck idled like shit for the longest time and I didnt realize it until I added a vented catch can for my PCV valve and capped the vacuum fitting.

    If the valve is bad, sometimes spraying inside it with gunk cleaner will help it but eventually it will need to be replaced. Dont get the autozone crap, go with the OEM and pay the $$$.

    If there is a bad neg ground, it can affect the ignition system and other electrical systems. If its very bad, it can possibly effect the spark.

    I have a mechanic friend who has a tester for determining if your head gasket leaks. You out a chemical into a bottle and hold it on the rad. It detects exhaust fluid. Didn't come up with any exhaust so I don't think it's head gasket

    I would say motor is done cyl one has 60 psi all the rear have 120
    Leak down test on cyl one coils hold barely no pressure so the valves are toast
    Time for a new motor
