02 sensor reading bad most of the time. As simple as the gas cap loose sometimes. My 4Runner had that the whole 4 years I had it and it was either bad O2 or CAT.

Bad gas shouldn't show those codes...

Well, I filled up the day it happened(Wednesday). They came on about 15 miles after fill up. I tried to reset it myself (battery disconnect) but it came back(Thursday morning). By the time I got by the dealer I was at 1/4 tank(Today, Friday).

So I left the dealer and bought a can of SeaFoam(lights were reset at dealer), I am now one can of seafoam and 2 full tanks of gas later and they have not returned.

Also, my MPG had dropped from my normal 18mpg to 16mpg. By all indications, it was just bad gas. Unless it comes on tomorrow.

PS: Yes I filled up full twice today, I drove a long way.

Do you need to know, or care?
I'm always careful where I buy gas, but I guess even quality stations could have their issues.

A back step for when I get older, Carhartt seatcovers, 4Runner wheels, Topper, and "tats all folks"! --for now!!
I found out the hard way that it does not take much of a leaky gas cap to set these error codes. For 5 months I had a locking gas cap--no problems. then all of a sudden late this summer those codes popped up. Dealer said that cap was it --of course I did not believe them as I had ran with the locking cap for 4-5 months. I put the OEM cap on the prob stayed away for 2 tanks. Tried the other cap again --prob came back --this time I had a mechanic friend reset it back. I did not want to "eat crow and go back to the dealer.
Make sure you are getting your cap tight --or maybe you have a bad cap?

Ha, we probably had the same gas cap! And it was advertised as NO CELs and it too worked fine for a couple of months but after I left my truck in an outdoor parking structure in the late summer for a couple of days, I hopped in and my dash was Christmas lights! Changed back to OEM and cleared the codes with OBD Fusion, but the “permanent” codes hung around for a few weeks until enough drive cycles completed for it to clear them.

Even the Toyota techstream can't clear the permanent codes because they are "permanent"

These locking gas caps are hit-and-miss. I have tried two brands, one of which I was told was junk, but it turns out that was the only one that sealed and did not give me the check engine light. Standt brand is what I am using.

right, but so long as the concern has been corrected it will clear itself after a lot of driving cycles. I probably had 100 warm ups before mine cleared from that damn gas cap that's about to go in the recycling bin...

No, First he thought gas cap, then they thought fuel pump or injector issue. Everything checked out. After 2 hours. Bad gas.

The speed limit is 75, I'm not going that fast.... ;-)
2 lane road, no one around. Yeah, I took a picture while driving. I have had my truck 9 weeks. If I waited to stop I never would have taken it. I'm at 11K miles today.

I've only ever experienced two things with bad gas:

    1) paranoids being careful where they buy gas
    2) toyota dealerships failing to diagnose problems
