I actually just watched a video on this a few days ago though I think it was geared more towards 10th to 16th century clothing. Usually the male vs female distinction was where the belt was tied and even those were universally sized and meant to drape or wrap the excess. Headdresses, corsets, more sleeved undergarments was an easy way to turn the same men's dress into a women's.
