    I have a 2009 4x4 TRD V6 @ 180K mi, that does the exact same thing. Took it to the dealership and was told "no idea". There are no codes, so they are lost.

    Was the problem ever fixed/solved?

    My feeling it is temperature related. The engine will still be warm after 30 min but a sensor may tell the computer different. Upon starting, it may do a cold start and add too much fuel.
    I think mine has done something similar but to a lesser degree.
    I could be totally off my rocker though.

    Maybe. But i took it into the dealership and there were no error codes. Doesn't a failed sensor usually show up on the diags?

    The sensor likely not failed. I'm suggesting a sensor was lying or was misinterpreted. Just a haunch. Also just a random glitch.
