As a quick deal pull the access to the cabin air filter and just look in there to see if there has been any debris get in there. I have seen that on more than one occasion. Also have seen squirrels make nests in there or mice etc. It's ironic when a squirrel does it because the name of the fan itself in the industry is a squirrel cage.

This one does not it keeps the stock location of the bolt but uses a smaller dia. bushing it appears and just moves where the cab mount is welded to the frame.

One time I had a bird take up residence in the exhaust pipe of a bathroom fan. It eventually died. Well wouldn't you know it, he died in such a position so that the tip of his beak was just catching the blades of the bath fan. This sounds exactly like that

Acorns, sticks, insulation etc. I have seen it all down in there sometimes it's just from falling through the fresh air vents sometimes animals put it there. My folks had a squirrel chew the wires in their 99 tacoma and relocate them to the blower fan for nesting material. Took me almost a whole day to repair their wiring harness.

Pull the blower motor out from under the glovebox. Gurantee there is something in it, leaves, dead bug, some sort of foreign object.

The cabin air filter is probably also hideous
