I assume this is what's called slipping (yeah that's my knowledge level). When driving up hill sometimes the RPMs will start rising quickly with no change in speed. If I back off the gas it will normally sync back up, sometimes I have to shift to get it to fix. Then yesterday just driving along on the highway the RPMS just start racing. I down shifted to fourth and drove the rest of the way normal more or less. I'm going to take it to the transmission shop today.

Is this slipping, what's the likely cause, what's the likely cure? (Is my tacoma totalled?)

Some bumpkin searching is suggesting this is just my clutch and not my transmission. Anyone agree or disagree?

Clutch is definitely shot!! Those symptoms are 100% clutch slipping/used up!! You'll need a new clutch and you'll need to resurface your fly wheel cause with all the slipping you let happen its surly glossed up and has hot spots all over it. So if its not resurfaced the new clutch will be in danger of slipping or atleast not having anywhere near the holding power/friction it should have!

Considering the age of your truck and if you plan to keep it , go with a new clutch master cylinder , a new clutch slave cylinder and a fresh shifter bushing to assist with your new clutch . Spend some time sniffing around the Marlin Crawler website for your new clutch . A lot of members at this forum use Marlin Crawler clutches with great success .
