You would have to get 2 of them, or get 1 of these for the relay, and the base that you can put wires directly into without needing to crimp anything.

I deal with those types of relays at work a lot, except with 24VAC coils instead of 12VDC.

You'll need 2 relays and 2 bases. Both relays will be 2 pole. Can either terminate wires directly under set screws on relay bases or use stak ons

If somebody can come up with a PnP kit, I think they'd have a gold mine. I just read the entire thread(again) and it seems no one has confirmed to have successfully done this on a 12 yet? Great write up. A perfect example of TW members helping members.

Nobody has gotten this on anything later than 09 is my understanding. I gave it in mind to try on my 11, but it isn't on my to do list anytime soon.

These trucks are damned complicated when it comes to wiring IMO. This would be by far my favorite mod, but I haven't had the balls to even start buying parts. I'm sure it's less complicated than I think, but I don't even know what I'm looking at when seeing the diagram in the OP.

Parts are easy. Some red and black wire, add a fuse thing from auto parts store, 4 pole double throw relay (or two DPDT relays) with 12VDC coil, and switch to mount in your truck in easy reach of where you drive, and likely a few female disconnect crimps. Put in add a fuse right on battery positive lead, splice to red wire. Put black wire onto negative of battery. Run black and red wires in through firewall into cab. Red wire to switch (if switch has a light, put black piece off of it, and splice to piece coming in from battery, and another piece that goes to other side of truck behind glove box) and another red from switch to other side of truck behind glove box. Red from switch to one point on coil of relay, black to other side of coil, as well as a couple points for normally open on relay.
