    What was the diagnosis?
Those codes and symptoms point to bad injector driver in the ECM.

I hope for OPs sake I am wrong, but I dont think all that needed changed and the problem may not be resolved completely.

Your results will vary depending on who worked on the truck, a mechanic or a parts changer.

I don’t get all the critique going on here now as to how the OP problem was address by the dealer and TOYOTA.

Damned if you do damned if you don’t geez!!

More than likely they found one faulty injector and a leaking gasket and wisely decided that being a new vehicle, the best approach to avoid a re-occurrence of the issue was by replacing all the associated components in the system, and turn the chapter on the story.

Moreover, it’s a lot cheaper labor wise to replace the components at once while everything’s already in state of disassembly, rather than risk another component failure occurrence in the associated system(s)

    Cheers!

    Best response here.

You see... Here is the problem with why everyone hates dealers. From tech to the person on the phone, there's probably 5 people involved. If you want to know the absolute knowledge of what's done, talk to the tech!

And of course on the https://www.toptiergas.com/ website where “Vehicle and equipment manufacturers write and enforce the standards”.

Although it wasn’t written on my repair invoice, the service writer hinted there was indeed a “leak of some sort” and that was why the 6 direct injectors and 2 gaskets were replaced. Drove it home yesterday (a good 35 miles) and it ran fine. Happy to have it back. Just still confused as to why it happened other than a component failure.....
Thanks all for your comments and opinions....

I don't think you did anything wrong or operated the vehicle in any way that would cause a problem.
I do short trips or multiple errands all the time with no issues.

If they would have just replaced one injector and a few days or months later another one went out, it would cost many expensive labor hrs to do the job again. And lets not mention they need to get you another vehicle at their expense or the inconvenience it puts on you. And some companies do care about their reputation and your satisfaction.

Glad it all worked out for you

    Cheers!
