Anyone run this set up? Every where I see photos they have the TSB leaf in, I only have 3 leafs and would like to avoid a nose high look thanks

I've installed this setup on a few friend's trucks. You should be fine. If it is a little nose-high, you can always slip in a spacer in the rear.

How does a rear spacer for back work? I’ve seen them the smallest in 1 inch, that would be 2.5 in the rear and the Bilstein 5100 are only rated for 1 inch right?

I bascially have 2.5' lift in the front and have an after-market HD progressive 3 leaf pack on top of the stock overload (so 3leaf + overload) and my truck sits fairly level with an ever so slight positive rake.
