For what it's worth, I've been fighting this same stink for about a month now. I tried one of those bug bomb type air fresheners a couple of weeks ago, where you let the system run on high, recirculate while the can does it's thing in the cab, and it helped a little.
Yesterday, I removed the glove box and pulled the cabin air filter. Then I hosed Lysol Nutra Air into the filter opening for about 30 seconds while the A/C ran on high, recirc. I then let it run for about 20 minutes and buttoned everything back up.
Today, there's no mildew smell. I read another post on here about about the Lysol thing with similar results. It's cheap and worth a try!

I've read a lot of people using lysol and I've read a few bad experiences like the smell not going away for months. I ended up picking up the Subaru kit, it was the cheapest I could find and it arrived yesterday. The instructions seem slightly different from the Toyota can, it says to use the entire foam can whereas I think Toyota recommends using half. I'll give it a try and post my results
