How often Do I need to change the fuel filter? I have an 06 DC with 125K miles and where is the filter located?

It's located in the gas tank, like all cars nowadays.
I heard it's never suppose to be changed....

    From Toyota's website

    For 2nd gens.
The fuel filter is typically not a scheduled maintenance item and only requires replacement if necessary. Please consult with your local dealer for additional information concerning fuel filter maintenance.

To change the filter you have to change the whole pump assembly. Toyota made the pump non-serviceable. Its much cheaper to get a walbro pump from URD which puts the filter outside the tank.

That's what I hear but some folks say to replace it every 15, 50k I read that toyota made it so well so you don't have to mess with the pressurized fuel system and it is not a scheduled maintenance item.
