I have a DC SR5 Prerunner and would like to get some Bilstein offroad components (shocks & struts/springs). It currently have the stock 3 leafs. It no longer qualifies for the TSB, too many miles now at 43k. Do I need to add another leaf in order to use the rear Bilstein shocks?

Nope, you can use the shocks with any stock setup. All of the 6 lug Tacoma suspension components are interchangeable. If using the 5100 series rear shocks, get the stock length shock.

No, we have the same truck I'm running that set-up minus the off road's coils.

Notice a big difference in your ride? Any reason you chose not to get the front off-road coils?

^^I got a firmer ride but also noticed a little bump steer when going over rough terrain, I still prefer the Bilsteins over the non TRD shocks though. The Sr5 has the TRD Sport linear coils and stabilizer bar, the TRD Off Road has progressive coils and a smaller stabilizer bar therefore it was easier for me to upgrade to the TRD Sport suspension. If I wanted the TRD Off Road suspension I would not only have to change the front coils but also the stabilizer bar. I didn't want to experiment with a mismatch (TRD Off Road coils with TRD Sport stabilizer bar).

Didn't know that about the stabilizer bar, thanks for the info. :D Looks like I'll be looking for just the TRD struts & shocks.
