You might have a bad head gasket. Have you done a compression check yet?

What vehicle are we talking about?
I would check the radiator cap and pressurize the cooling system to be 100% certain you don't have a coolant leak.
Does this vehicle have a fan clutch? Is your fan belt tight?

96 2.7 manual 4X4, Fan RPM is good and I checked the belt tightness when I replaced the water pump. There's no external coolant leak so as you said; need to do a pressure test to see if head gasket or something is cracked.

Had this on my Honda civic: oil in the coolant, bubbles when the radiator cap was off and overheating. It ended up being a cracked block. Like others mentioned do a pressure check and inspect the coolant for contamination.

As stated check for head gasket, also don't be convinced fan clutch is good just cause fan turns. I chased a fan clutch for a month. Was a bitch
