That’s definitely a possibility and would make the most sense. I’m gonna check it out today. Already ordered ANOTHER new belt

    Yeah.
That fan pulley looks crooked to me, just hard to tell from the pics.

Is your routing on the money? I went dyslexic putting mine on the first time.

    Too few to list.
I always take pictures before I take the belt off. Just because it fits doesn't mean it's on the right way.

I’ll post up some more when I get the skid off. Jumped in the truck this morning to go to work and heard a slight squeak. Got under and looked and knew right away that it slipped

    Yes belt routing is good.

Used the diagram to verify when I put it on

BUCKLE UP! It makes it harder for Aliens to pull you out of your Truck.
Is it a Toyota belt or aftermarket?

It's easy to not have the fan and pulley seated properly if you took those nuts off while the belt was still on the engine. Try redoing the nuts on the pulley after making sure it's seated.

Well it looks like the crank pulley is sitting out too far. Replaced this about one year ago (dayco)

I have not had to pull the crankshaft pulley/harmonic balancer on a 1GR-FE. From what I can gather you need a puller to get it off - doesn't slide off like some dampers do. If that's the case you likely had to use a bolt - same thread pitch as the crank pulley bolt but longer and a spacer with some shims to get it back on.
How did that go? Assuming Dayco part is correct.
I'm a little baffled as to how you went a yr. with the crank pulley out of alignment or was this recent? - crank pulley bolt loosening? - or is something else out of alignment?
    Puzzled.

It's a harmonic balancer. Steel/rubber/steel. It has shifted between the steel and rubber.

When I installed the crank pulley it was fine. For some reason it is now sticking out 1/8” or so.

Do not need a pulled to remove it. However I did have to use a holder tool and a torque wrench to install

I actually changed it twice. First time i went with a rock auto brand and it had a slight wobble. Got the more expensive dayco one.

Top pic is a year ago and bottom pic is now

I guess not? You replaced one with a Dayco a yr. ago. I was trying to dig thru the FSM doc made available here for '05-'06. Not much popped up for crankshaft pulley removal. I dug deeper and saw a diagram in one section that showed the crankshaft pulley, but found no removal doc. I know it's there - just not popping up. I googled the issue and saw several comments about a puller being used on a 1GR-FE engine.
But the issue now would be why you are off approx. 1/8". That would be related more to installation vs. removal. If it were my truck, I would lock the crankshaft pulley and re-torque the bolt. If that doesn't not help I would wonder about the fit of the Dayco product. In saying all of this I am assuming there is an issue with the crankshaft pulley and not somewhere else.
