Exactly how bad would it be to run wheel spacers so that you wouldn't have to do any trimming if you go with the 285/75/16's on the OME 3" lift?

There are many people that use them without any problems. However, there are just as many that will tell you not to use them. They do put more stress on wheel studs, hub bearings, and other parts. It's your ride... Do what you want, just remember that if you save money not buying a lift or wheels that fit correctly, you may have to spend money on other issues. Depends on what you use your truck for. If it is your daily driver, think reliability. If it is a toy, you are probably already prepared to break stuff.
